For those of you that are thinking of resigning from the LDS Church, here is a sample letter that you are free to use and adapt to your needs. You may add any other thoughts that might suit your particular situation.
This is to inform you that I am hereby requesting that my name, ________________________, (list wife and children also by name) be stricken form the records of the Church of Jesus Christ of Latter-day Saints. This request is made freely and with full understanding of the consequences of the act, as is taught by the Church.
I must insist that the record, and the letter notifying me that my name has been removed from the records of the church, show that the only reason that my name has been removed from the church records is that I requested it to be so. I also insist that the word 'excommunication' not be used in the letter notifying me of this action. Please be advised that I am taking this action because it has been demonstrated to my satisfaction that Mormonism is in great error, and teaches false doctrines.
I do not place my eternal life in the hands of any latter-day prophet nor is it (eternal life) conditional on my membership in any church or priesthood. I place my life at the foot of the cross.
I do not believe Joseph Smith was a prophet of God. He fails every test of a prophet including leading people after another god, uttering false prophecies, teaching false doctrines, and lying.
I am convinced that the Book of Mormon is a contrived document and not of divine origin.
My family and I rejoice that I have recieved the real Jesus Christ as my personal Lord and Savior, and that I have the assurance and joy of my salvation according to God's Word, The Holy Bible (1 John 5:11-13).
Please be assured that my decision has been the result of much prayer, and is final. I will consider any attempts by Home Teachers, Visiting Teachers or other representatives of the Church to contact me as an infringement of my free agency.
I will not participate in any Church court or "trial," for I have done no wrong.
Finally, I testify to you that Jesus Christ in THE mediator between God and man (1 Timothy 2:5), and the Mormon's apostles, prophets and priests are false ministers (2 Corinthians 11:13), and that Mormonism is the "other" (i.e. "false") gospel that Paul warned of in Galatians 1:8-9).
Jesus Christ declared, "I am the way, the truth and the life, and no man cometh unto the father but by me." (John 14:6). I have chosen this day to follow Him!
You should send a copy to your Stake President and also to the Presiding Bishoprics Office in Salt Lake City, Utah.
